You are caring for a 50-year-old male patient recently diagnosed with Type II Diabetes when he presented to the Emergency Room with a wound on his plantar area of the left foot, that “started as a blister from his shoes rubbing” and then according to the patient, “
Pt’s serum glucose on admission was 390 and the HGA1C: 12.
MD orders
1800 cal ADA NCS, 2Gm Na+ low chol diet
Metformin 500mg PO BID with meals
Glipizide 5mg PO every AM
Continue anitHTN meds from home
simvastatin 20mg PO daily
Vancomycin 1Gm / 250ml D5W IVPB to infuse every 12 hours
Flagyl 500mg / 100ml D5W IVPB to infuse every 8 hours
Rocephin 1GM / 100ml D5W IVPB to infuse every 24hours
Lispro Insulin 5units subcutaneously with each meal (TID)
Lantus 20units subcutaneously @ 0800am daily
Blood sugar monitoring: AC and HS with Humulin Regular Insulin SQ Sliding Scale
<150 no insulin
150-199 2 units
200-249 4units
250-299 6units
300-349 8 units
350-359 10 units
360-400 12 units
