Latisha Jones is an 18 month old female, brought to the Emergency Department of II Hospital for shortness of breath. Patient is sitting on her mother's lap in obvious respiratory distress. She appears to be well-nourished and normal size for her age. Subjective: Mother states that the child was eating and drinking as normal yesterday but refused any food or drink today. She has been much less active than usual. She noticed that Latisha was showing some signs of respiratory distress and is drooling. Objective: Patient is not interacting with staff and keeps dozing off. When her head starts to droop to sleep, she is aroused and takes in a large breath. This happens several times while you are in the room to assess the patient. She also bobs her head every time she takes a breath. She has substernal and intercostal retractions and her nail beds are slightly blue. Physical findings: Heart rate 126 • Temp (rectal)- 39C Decniratory rate 42 and labored 8:33 AM O
• Breath sounds - decreased with stridor SpO2 = 88% on room air. No other labs or imaging yet ordered. The ED doctor just graduated yesterday and has no idea what to do. You are asked to make recommendations. What specifically will you do? What should the doctor order?
