150 mL (when mixed) Sample antibiotic for oral suspension 125 mg per 5 mL CAUTION—Federal (USA) law prohibits dispensing without prescription. Prepare a 200 mg dose. Usual dose --Pediatric patients 20 mg per kg a day (40 mg per kg in otitis media) in three divided doses. Adults 250 mg three times a day. See literature for complete dosage information Prior to mixing, store at controlled room temperature 59° to 86°F (15° to 30°C). Directions for mixing-Add 90 mL of water in two portions to the dry mixture in the bottle. Shake well after each addition. Each 5 mL (approx. 1 teaspoonful) will then contain antibiotic suspension equivalent to 125 mg.
