Based on your patient's medical diagnosis you need to list 3 Nursing Diagnosis that are appropriate (1 based for pathophysiology of their diagnosis 1 based for nursing care and 1 for patient education.)
Outcomes: Based on your patient's nursing diagnosis you will have 3 outcomes/goals. These outcomes must have a Noun (patient/caregiver etc), an action (what you taught or what intervention was performed etc.), and a time frame ( within 2 hours, prior to discharge, during hospitalization etc.)
Interventions: This an action you are taking based off the nursing diagnosis. ** An evidence based research article must be used to support 1 of your nursing interventions and you should utilize in-text citation for this.
Rationale: This should be based on evidence based research WHY are you the intervention you chose is being completed
Evaluation: After the intervention is complete how will you evaluate that intervention to see if there is improvement?
