Case#1
A 34 year old female is admitted in active labor during week 39
of pregnancy. She had a previous cesarean section 2 years ago
as a result of fetal distress and cephalopelvic disproportion
(CPD). No fetal distress was found during this admission but
it was determined that the patient still had cephalopelvic
disproportion, therefore a repeat low cervical cesarean section had
to be performed. A healthy 8lb 10oz female was safely
delivered.
Diagnosis: Delivery complicated by Cephalopelvic
disproportion (Repeat Cesarean Section)
Procedure: Cesarean Delivery (Root Operation is Extraction
Obstetric)

Case#2
The patient is a 30 year old woman, gravida 3, para 2 in week 40
of pregnancy. The patient is admitted in labor. The
labor proceeded normally and the woman delivered a healthy male
infant vaginally without complications. The mother and baby
were able to be discharged on the hospital day 2.
Discharge Diagnosis: Normal spontaneous vaginal
delivery; Weeks of gestation; Outcome of delivery
