11:33 10:47 Scenario A A 26 year old woman in your facility went to the Operating Room to have an appendectomy. Postoperatively, the patient became septic and required further surgical intervention. Upon returning to the OR, it was discovered that not only had her appendix not been removed, but it had subsequently ruptured, and her uterus, ovaries and fallopian tubes had been removed in the first procedure. You are a Nurse Leader in this facility. As such, you have been charged with leading the investigation into how and why this happened. Use this scenario to respond to the following questions. 1. How would this event be classified? Include why it is classified as such, what organization created the designation and what requirements must be followed, including timelines. 2. What guiding principle is required in such an investigation to foster a positive working environment and prevent future events from being covered up? 3.
