Mrs. B presents to the ER complaining of fever, severe back pain and dysuria. A urinalysis reveals many WCs, RCS and red cell casts. Her admitting CBC reveals a 40,000/uL white count and 10.0 g/dL hemoglobin. With this news, the physician requests blood cultures. She is started on IV antibiotic therapy and is admitted to the hospital. 1. What is the admitting diagnosis? 2. For what complication do the blood cultures evaluate? 3. The white count is very high for this diagnosis. Is a bone marrow in order? Is this a leukemoid reaction? Explain. 4. The cell counter reflects a manual differential to be.performed. No blasts are seen on the smear, but some immature cells are noted. Which ones? 5. The Hematology tech also notices cytoplasmic abnormalities in the segs and bands. Two of these abnormalities are graded and reported on the CBC report. Name them.
