He has a past medical history of bladder cancer and high blood pressure. A physical assessment on the patient reveals the patient is alert and oriented, heart sounds normal, all lung fields clear, hypoactive bowel sounds, his abdomen is painful and you can feel that his bladder is distended. A bladder scan reveals 850 mL of fluid in his bladder. Highlight the assessment findings that require immediate intervention by the nurse.
