ase Scenario: N. is a 67-year-old male complaining of abdominal pain and the inability to void. He verbalizes not inating for over 24 hours and he feels a lot of pressure in his lower abdomen. The last time he urinated was red in color. His vital signs include: HR 90, BP 178/88, RR 18, oxygen saturation 99% on room ,pain 10/10. He has a past medical history of bladder cancer and high blood pressure. A physical sessment on the patient reveals the patient is alert and oriented, heart sounds normal, all lung fields ear, hypoactive bowel sounds, his abdomen is painful and you can feel that his bladder is distended. A adder scan reveals 850 mL of fluid in his bladder. ghlight the assessment findings that require immediate intervention by the nurse.
