An 8 month old with Pseudomonas growing from a respiratory culture who weighs 6 kg is started on Gentamicin 15 mg IV q8 hours and Ceftazidime 300 mg IV q8h for treatment of his pneumonia. The following drug levels were collected. Peak of 6 mg/L and Trough of 0.4 mg/L. Goal for treatment of pneumonia is Peak of 8-10 mg/L and Trough of 0.5-1.5 mg/L.
