Case Study: Mrs Yvonne Stewart
Background
Mrs Yvonne Stewart is an 85-year-old woman who has presented to
a large rural hospital in NSW for an elective left total hip
replacement. Mrs Stewart is a retired Primary School teacher and
lives with her husband in a small remote town, 100kms drive from
the hospital in which the operation will occur.
After consultation with an Orthopaedic Surgeon, it has been
decided that the left hip needs to be replaced to improve Mrs
Stewart’s mobility and quality of life. Over the past five years
Mrs Stewart’s mobility has been worsening and the pain in her left
hip has been increasing.
6/03/2022 - 0845
Mrs Stewart and her husband arrive at the hospital. Mrs Stewart
is admitted clerically, the clerk askes Mrs Stewart if she will
allow an identification arm band to be put on, Mrs Stewart agrees.
Mrs & Mr Stewart are directed by administration staff to the
waiting area for surgery. As Mrs Stewart walks to the waiting area,
she is favouring her right leg, grimacing in pain, holding on to
her husband, and looks worried.
After a few minutes of waiting, Riley (a Registered Nurse (RN)
working in the pre-operative area) introduces their self to Mrs
Stewart and asks her to identify herself. Riley then introduces Mrs
Stewart to all the staff in the area. Riley asks Mrs Stewart to
come through to anaesthetic bay to prepare for the operation and
for Mr Stewart to wait in the waiting area whilst Mrs Stewart gets
changed. Riley provides Mrs Stewart with a patient gown, asks her
to change into the gown and offers privacy whilst changing.
Riley asks Alex the Student Nurse (SN) who is currently there on
their first-year work placement to complete an assessment on Mrs
Stewart and do several things for her. Alex is asked to:
• Introduce their self to Mr Stewart and take him to Mrs
Stewart; •
Introduce their self to Mrs Stewart and Mrs Stewart asks Alex to
call her Yvonne;
• Help Mrs Stewart to be comfortable in a chair beside the
bed;
• Undertake a full set of observations; and
• Collect information from Mrs Stewart to complete a full health
history.

Riley also reminds Alex to provide plenty of reassurance, and
maintain the ‘five moments of Hand Hygiene’,

Alex introduces their self and is performing their hand hygiene,
and asks Mrs Stewart for permission to collect her vital signs. Mrs
Stewart nods her head in agreement and extends her right arm.
Vital Observations as taken by Alex between 0915 & 0930
• BP 172/95 mmHg;
• HR 100 bpm, irregular and fluttering;
• RR 18 bpm;
• Temp 36.7°C (per axilla);
• SpO2 99% (RA).
• BGL = 4.3 mmol/l
6/03/2022 – 0930
Alex (SN) performs hand hygiene using Alcohol Based Hand Rub
(ABHR) and brings a chair close to the chair Mrs Stewart is sitting
in and sits at a 45-degree angle from Mrs Stewart.
SN: Yvonne, could you tell me why you have come to hospital
today?
Mrs Stewart: I am having an operation on my left hip.
SN: Are you allergic to any medications?
Mrs Stewart: Yes. I can’t have penicillin. It makes me very
sick.
SN: What happens when you have penicillin?
Mrs Stewart: I get a horrible rash all over my body and my face
swells up.
SN: O.K. I will note that down and make sure we don’t give you
that then! Do you have any previous medical problems? Mrs Stewart:
Yes, I have high blood pressure and my heart goes into funny
rhythms sometimes. I have taken my pills this morning with some
water, I hope that is alright.
SN: I can see in the documents you have brought with you that
you have a list of medications. The Doctor who is coming to admit
you will look at that list. They can also double-check that you
were right to take your pills this morning.
Mrs Stewart: Thank you for telling me that, I am on a couple of
medicines and will tell the Doctor.

SN: Do you smoke cigarettes?
Mrs Stewart: No, not anymore. I did when I was younger.
SN: How are you feeling now while you are sitting here, are you
in any pain?
Mrs Stewart: I am feeling a little lightheaded and I have not
had anything for my left hip pain today. I would like to get into
the bed if that is OK?
SN: Yes, I can help you do that. I will lower to bed so that is
it easier for you to sit on the side of the bed and you will then
be able to lie down. Do you need any help with your legs?
Mrs Stewart: Thank you, can you just support my left leg please
whilst I get on the bed and lie down? Alex assists Mrs Stewart onto
the bed.
SN: May I take your pulse and blood pressure again I
noticed you heart rate was a little irregular?
Mrs Stewart: Oh, it is ok? Yes, you can take it again.
Vital Observations retaken at 0940:
• HR 105bpm irregular and fluttering (as before);
• BP 170/80 mm/Hg
SN: Now that you are in bed, are you more comfortable, do you
feel as lightheaded? Can you tell me how much pain you are
experiencing in your left hip, if 0 is no pain and 10 is the worst
pain you have felt, what would you score your pain now?
Mrs Stewart: Oh, I suppose it is a 6 out of 10. Its better now
that I am lying down and I don’t feel as dizzy as I did before.
SN: Your pulse and B/P are similar to when they were taken
before. I must notify the Registered Nurse, this is protocol. What
is the last thing that you have had to eat or drink? And, what time
was that?
Mrs Stewart: I had some toast before I went to bed at 10 o’clock
last night and a glass of water at the same time. I was told I was
not supposed to have anything to eat or drink today but took my
pills with a small glass of water at 6.15 this morning.
SN: Thank you for that information, do you have any difficulties
with toileting? Incontinence, constipation, anything like that? Mrs
Stewart: No, I am pretty regular.
SN: O.K. Mrs Stewart. I will report back to the nurse I am
working with today. Are you happy to wait here just a few minutes,
and would you like me to get you a warm blanket?
Mrs. Stewart: Sure, the warm blanket would be nice, thank
you.
