The fourth assignment of this course will be an individual task
in which you will write an incident report.
This will be based on the incident report form you invented
during the class, but the idea is to transform this into paragraph
form. Alternatively, you can come up with your own scenario for an
incident/accident report and write about it, but it must be in
paragraphs with complete sentences (not just completing a
form).
You need to attach a completed incident report
form as well as a narrative
report detailing the
incident (in paragraphs). Please include them in the
same document either in PDF or Word format. The form and the report
must have the same incident, but obviously the way you write it
will be different.
Please review the notes from our class before beginning to write
this. It is a good idea to check the examples we saw during the
lesson (03 Teaching Plan > Week 11) as you will be able to
receive some ideas on how to structure your ideas and it will show
expectations for this type of task.
The deadline to complete this task is Wednesday 13
April 2022 at 1159pm. You
should aim for around 250 words for the
narrative report.

On 20th February 2022 at 3:00 pm, an accident involving
a member of our work team (Omar Obaid) occurred in Welding Shop C
which resulted in him receiving burn injuries.
Write an incident report about this incident and
complete the template below. You might need to fill in information
from your own imagination.

An incident report is a document that records specific details
of an unusual event that occurs. The event may or may not have
resulted in an accident/injury, but the report could help prevent
future incidents.
An incident report must be detailed and accurate and there are
three parts to creating a good incident report.
Part 1- Follow protocol
Part 2- Describing what happened
Part 3- Polishing the report
