A 28 year old female presents to the Emergency Department with an intense headache with blurring of vision. Over several weeks, she has experienced headaches and fatigue, of gradual onset. She states she feels "restless all the time," with some flushing and heat intolerance. She is single, works as a commercial artist, exercises occasionally, does not smoke or use recreational drugs, and rarely drinks alcohol. No medications, nutritional supplements or unusual dietary practices. No drug allergies. On examination, the patient is of slender build, cooperative and pleast, but appears uncomfortable. Blood pressure is 186/96 in each arm, with pulse 110 beats per minute. Height is 165 cm, Weight is 55 kg. Fundoscopic examination shows slight fullness at each optic disc, without hemorrhage. No cervical adenopathy; the thyroid is firm and slightly enlarged, fuller on the right than left. No murmur is heard on examination of the heart. Peripheral pulses are bounding and symmetric. There is no edema of the extremities, and no organomegaly on abdominal examination. Neurologic examination is remarkable for increased deep tendon reflexes and some loss of peripheral visual fields for each eye.
