A.O. is an 89-year-old woman with a long history of systolicheart failure following an acute myocardial infarction 5 yearsprior. She has had some activity intolerance but is now starting toexperience moderate to severe shortness of breath and mid-sternalchest discomfort with normal daily activities. Although thediscomfort resolves with cessation of activity, she has recentlyrequired assistance with her normal routine. A.O. has alsorecently noticed marked bilateral pedal edema. She is currentlybeing treated with digitalis (Lanoxin), furosemide (Lasix), KCl,and has sublingual nitroglycerin ordered prn. She has justbeen admitted to the hospital for acute exacerbation of her heartfailure. Echocardiogram reveals ejection fraction of 40% andchest x-ray reveals cardiomegaly.
