Your team was asked to help resolve this compliance issue: Too many coding errors are being found with E/M codes. The auditor found many upcodes (99213 should have been 99212) and several downcodes (99213 should have been billed as 99214). The coders claim that conflicting information is often present in the health record, requiring them to spend a lot of time querying the doctors and nurses about their documentation. 1.
