An 80 year old grandmother GM (5'6 and 180 lbs) and her 3 year old grand-daughter GD (2'10 and 30 lbs) were admitted to Arkansas Children's Burn Unit after they were pulled from a burning house. They each had minor burns but significant inhalational injuries requiring both to be intubated. Three days after admission, both patients continue to have respiratory decline and develop fever. Sputum cultures are obtained. The physician is worried about MRSA and wants to start Vancomycin. GD's renal function today is BUN/Scr of 7/0.3 and GM's renal function shows BUN/Scr of 30/1.8. Based on known clearance for each, who would need more frequent dosing of vancomycin? (1 Point) GM

If phenytoin were needed in both GM and GD and it was given via a nasogastric tube, who will need a smaller dose based on metabolism? (1 Point) GD GM ... If phenytoin were needed in both GM and GD and it was given via a nasogastric tube, who will need a smaller dose based on having a decreased albumin status and increased free fraction? (1 Point) GD GM If phenytoin were needed in both GM and GD and it was given via a nasogastric tube, who would have the slower gut motility allowing for increased absorption if given away from enteral feeds?
