A 270 lb 40 year old man presents to the emergency room with fever, pain on urination, and lower back pain and is admitted for possible bacteremia and a urinary tract infection. His initial BUN/Scr are 28/2. The physician would like to initiate gentamicin. What gentamicin dosing regimen (dose and interval) would you recommend?
