- critical thinking scenarios These scenarios are put in place
to assess your thinking and teach you to use the knowledge you have
to provide care . There is not always going to be a right or wrong
answer as long as you have a valid reason for each answer
Question 1
1. You start your shift today and the in charge nurse lets you
know that you will be working short staffed. The following clients
have now been assigned to your workload. In what order will you
care for these clients? Provide a rationale (reason) for each
client A: suffered a stroke with right sided hemiplegia, this
client also has aphasia. She prefers to get up quite early. She is
a 2pa but can do some care with minimal assistance.
client B: suffers from moderate dementia. He is a fall risk and
needs regular supervision as he does not stay when seated. He is
completely ambulatory but needs cueing and assistance for care.
client C: is a mechanical lift with significant contractures. He
requires full care including needing to be fed. He sits in a
reclining wheelchair for the whole day and requires regular
toileting due to bowel and bladder incontinence.
client D: This client has MS. She requires the use of a
mechanical lift. She is full care. Once in her power wheelchair she
leaves the facility and does not return until late until the
night.

2. You arrive to your first day in a new client's home. On
arrival the Health Care Aid you are orientating with shows you how
to do a suppository for the client. You have not had any additional
training. The Health Care Aid says to you "it doesn't really matter
if the nurse shows you. We just do it anyways". The client states
that she doesn't like having tons of people in her house and being
used as an example. She would rather you just do the skill and get
it over with.
A. What is wrong with this situation? (list 3 things)
B. How could you approach this with the client and the
staff?
C. What communication techniques can you use to ensure you are
comfortable?
D. What type of supervision is required and who is to supervise
