An order is written for amoxicillin 250 mg/5 mL, take 5 mL every
12 hours. The pharmacy is out of the 250 mg/5 mL concentration, but
the 125 mg/5 mL, 200 mg/5 mL, and 400 mg/5 mL strengths are in
stock. What would you do? Would you use another strength or wait to
order in the specified strength? What directions would you use if
another strength is selected? What is one way to ensure the correct
amount is given to the patient, after it is dispensed? Should the
patient use a kitchen spoon or another device? What examples do you
have from your externship on tools that are used to help ensure
patients receive the proper dose of medications (either in the
community pharmacy setting or institutional pharmacy
setting)?
