Nursing Care Plan Assignment 20%
Purpose: To provide an opportunity for students
to use the nursing process and develop a nursing care plan for a
client.
Case Study: Jane Jones
You are a public health / community health nurse.
This patient was referred to your department for follow up at home
by an Emergency Department physician and discharge RN after the
patient was seen and treated in ED for exhaustion. This
patient is now assigned to you.
Jane Jones is a 28 year old woman who fled an abusive
relationship one year ago. She came from Albert to start afresh in
the Niagara region. Jane is renting a room from a good friend
for the time being. Jane almost finished high school; she is
missing two courses but plans to get her GED. Currently, she
works at a restaurant as a server. She works long hours
because she wants to save money to go to college.
Jane currently does not have a primary care physician and would
like to find one. When she has needed medical care, she goes
to the emergency department or the walk-in clinic and sees a
different doctor each time. Jane has asthma and the grease and
spices in the kitchen of the restaurant make it difficult for her
to breathe. Jane’s co-workers are all smokers, so she
breathes in second-hand smoke during break time. Jane has a
persistent cough as a result.
In order to save money, Jane either walks or cycles to work and
she finds it difficult to do so with her cough. In addition,
Jane is 5’ 9’’ and weighs 105 pounds. Jane says she is
usually too tired to eat before she falls asleep at night, so she
only eats whatever the restaurant has for leftover since it is
free.
Furthermore, Jane frequently has horrible nightmares about her
abusive past. Jane fears that her ex will find her and hurt
her again even though there is a restraining order. Jane
misses her friends and families in Alberta and only speaks to them
occasionally over the phone. Jane is afraid to be on any
social media in case her ex will locate her.
