Document a SOAP nursing note in handwriting based on the
following scenario;
Your client has returned to the clinic to check on their blood
pressure management. The patient tells you they will decrease the
amount of salt they eat when they see the dietician. The patient
has brought their home BP reading book, only measurements 2 hours
after antihypertensive medication administration have been
documented. The nurse completes teaching re: home BP measurements
(add details about your teaching here). After your teaching the
patient states “ I understand much better how I am supposed to
write down my pressures in my book.” The client agrees to (write in
here how the patient will now monitor their home BP reading
schedule after your education).
- Remember to include all elements in the handwritten note.
- You misunderstood the patient and have made an error in the
note. For the electronic note, go back in the record and correct
it, “the patient has decreased the amount of salt they eat after
they saw the dietician”. For the hard copy, handwritten note,
correct the error
