Scenario Mr. Bigman is a 53 year old male. He presents for his surgical out-patient clinic appointment where he is to be screened for right inguinal hernia repair. Upon assessment Mr. Bigman reveals that he has occasional headaches and chest discomfort, but he doesn't worry about these feelings because it isn't that bad. He admits to drinking alcohol and smoking cigarettes. You observe that he is obviously very overweight. His vital signs are T 36.2, pulse 94, RR 20, BP 185/102, SpO2 98%. Mr. Bigman returns to hospital two weeks later complaining of chest pains which started while he was weeding the garden. His vital signs are T 36.8, pulse 104, RR 22, BP 190/108, SpO2 98%.
