2 DAYS AFTER ADMISSION The woman continued to experience respiratory distress. When questioned, the patient indicated that she hid felt short of breath for several hours. She appeared cyanotÃc, was pursed-lip breathing, and was using her accessory muscles of respiration. She continued to cough frequently and produced large amounts of foul smelling, bloodstreaked, yellowgreen sputum. The sputumn culture revealed Streptococcus pneumoniae and Pseudomonas aeruginosa. Her vital signs were as follows: blood pressure 188/95, heart rate 118 bpm, respiratory rate 34/min, and oral temperature 37 C 198.69 F). Palpation of the chest was not remarkable. Percussion revealed flatness over the right lower lobe. On auscultation bilaterat rhonchi and crackles could be heard over the lung bases Bronchial breath sounds were audible over the right lower tung base. Her oxygen saturation measured by pulse oximetry (SpO2) was 93% Her ABGs were as follows: pH 7:54, PaCo2 30 mmHg.
