Case study The gestational age of the 6-hour-old newborn is 28W+3d, and the birth weight is 980g. He was admitted to the hospital because of "progressive breathing difficulties for 5 hours". The child developed dyspnea 5 hours before admission (i.e. 1 hour after birth), manifested as shortness of breath, grunting, and cyanosis. > (1) Why does the child have progressive dyspnea? > (2) Why do grunting occur?
. a There is a newborn , gestational age 28+4 weeks, who's birth weight is720g.He was born by cesarean section due to "intrauterine distress, placenta abruptio, premature rupture of membranes, and preterm labor" . Immediately after birth, he is given sputum, oxygen, positive pressure ventilation and other cardiopulmonary resuscitation 3 minutes after birth, his heart rate drops sharply from 140 beats/min to about 60 beats/min, with no spontaneous breathing and oxygen saturation of 48%, and he is immediately given epinephrine application and naloxone application. He was transferred to the neonatal unit for further treatment. Preliminary diagnosis:1. preterm infant 2. neonatal respiratory distress syndrome 3. neonatal asphyxia 4. extremely low birth weight infant 5. infant small than gestational age
