Medical Consultation: ENT Date: 6/16/2011 Patient: Shareena Mushreen Patient Complaint: Difficulty breathing, often with chest pain. History: 65-year-old female. Recent immigrant with no prior medical history available. Family History: Parents deceased 25 years; no siblings reported. Allergies: None Physical Examination: Vital signs abnormal: bp 132/95, pulse 81/min., temp. 99.7°F. Pulse oximetry and pulmo- nary function test indicate hypoxemia. Auscultation positive for pneumonia. Acid-fast bacilli negative. HIV test negative. Blood culture positive for Pneumocystis jiroveci. Diagnosis: Pneumonia with HIV negative.
