Past Medical History He reported medical history of hypertension, hypothyroidism, and gout. Family History Reports coronary disease from both parents. Social History He never smoked and reported a healthy diet and exercise five times per week. His daughter stated that her father had been under extreme stress due to sudden accidental death of his son two weeks ago.
