NEED HELP ON THIS RIGHT NOW! VERY URGENT!
Use the provider’s dictation below to correctly document
the following blank physical exam.
Heart had regular rate and rhythm, lungs were clear to
auscultation. She had some mild epigastric tenderness to palpation
with guarding but no rebound or distension. Visual acuity was
20/20, no icterus. Head was normocephalic and atraumatic. Neck had
normal range of motion and was supple, extremities had no
tenderness or deformities. Back was nontender but with some mild
scoliosis noted. No cervical lymphadenopathy noted.
