What is the annual cost of premiums for Jordan? Case: Jordan is 55 years old. Refer to Jordan's healthcare insurance identification card located on the Preliminary, Preparatory Worksheet for the assignment In this past benefit year, Jordan saw an in-network primary care provider (PCP, a family practice physician) in January. The primary care provider again referred Jordan to a gastroenterologist for a colonoscopy. After an initial visit at the gastroenterologist's group practice in February, Jordan had the colonoscopy performed at an in- network ambulatory surgery center in March. Fortunately, this time, no abnormalities were found; however, the previous year, Jordan had had a malignant polyp. Jordan's PCP and gastroenterologist want Jordan to have a colonoscopy again this year because of Jordan's personal and family histories (father, three paternal uncles, one maternal uncle, and paternal grandfather all died of colon cancer). Jordan has a chronic skin condition for which Jordan annually sees a dermatologist to have a prescription renewed. Jordan saw the in-network dermatologist in September. The prescription for the daily medication is a common antibiotic for which the physician orders the generic equivalent. Jordan is a former high-school and college soccer player. Jordan currently exercises 60 to 90 minutes per day, alternating daily between cardiovascular and free- weight routines. Last year, Jordan's hip had started to catch" sporadically. This past benefit year Jordan saw a physical therapist twice, once in September and once in October. The physical therapist prescribed two sets of exercises to strengthen the muscles supporting the hip joint Jordan's hip continues to hurt and Jordan plans to again visit the PT in this benefit year.
