Alex is a 2 year old who came into the hospital with symptoms
of constant crying, delayed walking and elevated levels of lactate.
The baby was given an IV of glucose and his condition worsens. His
glucose levels are normal when tested but his lactate/pyruvate
ratio is 10 times normal values. What do you think is wrong with
Alex? How do you explain the elevated levels of lactate? Why
doesn’t he improve with the IV of glucose? What therapy would you
suggest for Alex? Would you keep him in the fasted or fed
state?
