Ahmed is a 19-year-old who is from RAK, but currently living and studying in Abu Dhabi – he was accepted into an environmental sciences program and will live in the university hostel for the duration of his studies. Ahmed was diagnosed with bipolar disorder on his 14th birthday, but has been receiving pharmacological agents to manage this disorder. Ahmed knows that compliance with taking these medications is very important in keeping him stable. He started to show some disruptive behavior, had very poor eating habits, spent most of his time in his hostel room and progressively missed his classes. One of his friends was very concerned and tried to reach out, but Ahmed refused to make any contact with him. Ahmed’s friend proceeded to inform the university student affairs body – upon investigation, they found Ahmed sleeping in his room, visibly he looked like he had lost a lot of weight and his prescription medication for bipolar medication was not used for the entire month. Emergency services was called and Ahmed was taken to the nearest hospital for further assessment and management.
Upon admission, Ahmed was found to be 1.72m (height) and weight recorded as 56.0kg. His medical records indicated that his UBW was 62kg (recorded about 4 months ago). Ahmed was very disoriented and was therefore unable to take orally. The team tried inserting an NGT but he kept pulling it out. As a result, a decision was taken to initiate TPN.
