You're a nurse in a facility that takes long term care residents and short-term rehab residents. You have admitted Mr. Anderson, a 54-year-old male resident, who is here for rehab post open heart surgery. He complains of having pain in his surgical and chest tube incision sites that he rates as a 10/10, despite interventions to help control his pain. His wife comes and visits him daily. You go and check on Mr. Anderson after he has had therapy with PT and OT. He is sleeping heavily in bed, and when you try to arouse him, he will not respond, even to noxious stimuli.
