Judith, a 29 year old mother of two, arrives at the emergency
room of the local district hospital via ambulance with exacerbation
of asthma after complaining of anxiety, dyspnea and severe
wheezing. She was discharged from here a few hours ago with a
similar complaint where she was treated with albuterol nebulizers
with good effect.
Following her discharge, she experienced tightness in her chest
and shortness of breath. She used her albuterol puffer several
times but got no relief. She was diagnosed with asthma three years
ago and she is allergic to cigarette smoke. She does not have a
primary physician and does not take any other medications.
Assessment findings in the ER reveal the following:
Sitting upright and using accessory muscles to
breathe.
Talks in one-to three-word sentences
RR 36 and shallow; HR 126, O2 Saturation: 88%
Audible wheezing, and no air movement in lobes when
lungs are auscultated
Anxious and restless
Chest X-ray findings reveal bilateral lung
hyperinflation with lower lobe atelectasis
Blood work within normal limits.
An IV is started in her left forearm with normal saline infusing
at 100 ml/hr.
