Mr. S, a 36-year-old man was referred from a private hospital
with severe generalized abdominal pain of 6 hours duration and
profuse vomiting. His significance past surgical history is
laparoscopic cholecystectomy few years ago for cholelithiasis and
open appendicectomy 2 years prior. He had a similar attack one year
previously and was thought to have diverticulitis but it appeared
to resolve with antibiotics and he was not followed up.
On examination:
•
Blood pressure: 124/76 mmHg,
•
Pulse: 104 per min. and regular,
•
Respirations: 26 per min,
•
Temperature: 38 ºC.
Abdominal examination reveals generalized guarding and
tenderness all over the abdomen, and bowel sound absent.
