4) Nurse went to patient's room at 0800AM to assist with eating breakfast: 82yr Asian male who has bilateral cataracts and bilateral glaucoma. He has 60/80 vision due to Diabetic Retinopath and Hypertension. VS=T-98.0 P-82 R-22 B/P-130/96 O2 sat-98 Pain-0/10. He states, "can yo help me to get out of this bed to sit up in the chair to eat my breakfast". He is alert and orientated X4 and has a steady gait but needs guidance to chair. You (the nurse) set up his tray and inform him the clock position of food items. He independently feeds himself and eats 100% of Heart Healthy diet.
