A 70 year-old man with chronic bronchitis noticed swelling in
the right groin. On examination, attending physician noted
elongated swelling above the medial end of the right inguinal
ligament. The physician inserted a finger into the patient’s
scrotum superior to his right testis, and along the spermatic cord
to the superficial inguinal ring. When the patient coughed the
swelling enlarged. However, no impulse was felt on the tip of the
physician’s finger.
• What is the difference between the inguinal ligament vs
inguinal canal? How is the inguinal formed (general terms)?
• Explain why the physician did not feel pressure on his finger
at the site of superficial inguinal ring.
• Draw and label the area on the anterior abdominal wall where
the hernial sac protrudes through.
• How does an indirect inguinal hernia differ from a direct
inguinal hernia in presentation?
