A woman who had had two difficult vaginal births consulted her
doctor with urinary stress incontinence. She also complained of a
heaviness or pressure sensation in her pelvis. Gynecological
examination reveals a mild downward bulging of the anterior vaginal
wall, gentle pushing on which reveals urinary incontinence on
straining. The cervix is elongated and found to be located a bit
lower down in the vaginal than normally.
• In your opinion, what has happened in this case? What is the
cause of urinary incontinence?
• What organ is most likely to be bulging into the anterior
vaginal wall?
• What is the normal position of the uterus? What is the uterine
prolapse?
• What is the ‘normal’ size of the uterus?
• This woman was sent to see a specialist. In the meantime, she
