Medical Consultation: Ophthalmology Date: 10/15/2011 Patient: Salima Aziz Patient Complaint: Right eye pain, crusty exudate, right eye pruritis and swelling that has spread to the left eye. History: 16-year-old female with negative history for ophthalmology. Family History: Unknown Allergies: None Physical Examination: Slight fever of 99.5°F, other vital signs normal. Right eyelid and OU with signs of inflam- mation and exudate. Diagnosis: OU conjunctivitis, keratitis, and blepharitis.
