Absence of the following aquatic insects:

Absence of terrestrial insects in the vicinity of small streams
with weak currents and extensive overhanging vegetation
Absence of algae in riffle areas
Lack of leaf litter or tree needles
Absence of bird predators
Absence of fish predators such as:
Obstructions
An excess of number spawners or juveniles downstream or upstream
from rapids, logjam, culvert.
A partial logjam or any other partial obstruction
A logjam in the upper reach of a stream, beyond the migratory
range of fish
Landslide or Rockslide
Water through a culvert is flowing too fast.
Water in a culvert is too shallow.
Perched culvert – culvert place too high above stream
surface.
Culvert too long or too steeply inclined.
Culvert opening too small for the size of the fish in the
stream.
Streambank Conditions
Cattle grazing near the streambank.
Any other loss of riparian vegetation.
Fallen trees.
Tree blowdown.
Streamflow Conditions
Gravel bed is scoured by strong currents.
Gravel bed changed location.
Instream pools filled in with rocks and gravel.
Streambanks crumbling from the effects of strong currents.
Stream spilling over its banks.
Streambed stability reduced.
Instream Conditions
Sand and silt buildup on the stream bottom.
Ice buildup
Presence of fry in the stream before the time they normally
emerge from the gravel.
Excessive weeds.
Plankton blooms.
Presence of dead adults during the spawning off season, or dead
juveniles year-round. No signs of obstruction.
For each sign observed (explained in the first column) explain
the stream habitat degradation in detail and its threats to the
fish and fish habitat.
