A 58-year-old client with severe chronic obstructive pulmonary disease (COPD) developed an increased cough and dyspnea over the past week. Current client findings include: Lethargic but responds appropriately to direct questions • Vital signs: heart rate = 89 beats/min, respirations = 28 breaths/min, BP = 144/68 mm Hg, oxygen saturation : 85% (on room air (RA) • Wheezes present throughout bilateral lung fields Arterial blood gas (ABG) results: pH = 7.34, Paco,- 55 mm Hg, Pao - 68 mm Hg, HCO - 30 mEq/L (30 mmol/L) The nurse consults with the registered nurse and primary health care provider to develop a coordinated plan of care, which includes: Bi-level positive airway pressure (BiPAP) therapy Albuterol nebulizer Supplemental oxygen, 2 L/min After implementing care, the nurse reassesses the client. For each client finding, indicate if the client's condition has improved, has not changed, or has declined. Place an X in the appropriate space.
