Patient is status post left total hip
replacement secondary to localized osteoarthritis and was
experiencing problems with ambulation and gait following hip
surgery. The patient was discharged with home health services twice
weekly. Home care was ordered for wound care. The patient also
received physical therapy for gait training and strengthening to
