Dx: Huntington's disease
Hx: Poor Skin integrity: Skin tears and bruising
Long Term Facility: Resident
Mrs. Keratin remains in bed much of the time. Due to agitation, confusion and moving about in bed Mrs. Keratins has padded side rails to aid in preventing injury when coming in contact with the bed rails. During your shift you walk into Mrs. Keratin's room and discover that she has become trapped between the mattress and the
bed rail. You've observed that Mrs. Keratin has no apparent injuries, but you are unable to free her. You immediately call your supervisor and request assistance.
• Research the diagnosis; identify the etiology, and prognosis common signs
and symptoms, treatment options (in brief) and treatment of choice.
• Identify the primary components of the health challenge (i.e., pain, loss of
function, immobility, fatigue, confusion, stress..etc.).
• Identify how the above components affect the clients' health and Physical
well being
• Identify how the above components affect the clients' health and social well
being
• Identify how the above components affect the client's health and Emotional
well being
• Identify how the above components affect the clients' health and Spiritual
well being
• Identify how the above components affect the clients' health and Cognitive
well being
• Identify measures that you can take to help the client maintain as much
independence as they can for as long as they can
• Identify your role as HCA in caring for and supporting the client. List comfort
measures that you can provide to assist the individual in a sense of well
being
• Identify community resources, in your local area, applicable to the specific
diagnosis that you, the family or the client might use
• Develop a poster highlighting at least one of the identified community
resources
• Report presented professionally in a report folder with cover page and
research cited
