It is common for clerical errors to occur in the medical office,
but it is important those errors are handled in a quick, efficient
manner because the medical record is a legal document that can be
requested by the courts for legal cases. Sara Lee has been seeing
Dr. Crout for three years. Sara Lee requested a copy of her clinic
notes from her last two visits. After receiving and reviewing her
notes, Sara Lee noticed a couple of errors in her notes from one of
her visits. The first error she noticed was that her social
security number was inaccurate. The second error was that her
primary pharmacy stated Walgreens when it should be Publix
pharmacy. Furthermore, she notices several diagnoses in her Family
Health History section (both mother and paternal grandmother) that
she never reported to Dr. Crout’s staff. Sara Lee made the errors
known to the medical staff. Does Sara Lee have the right to ask the
medical staff to correct the errors?

For your initial post, explain if Sara Lee has the right to
request her medical documentation to be corrected in her chart.
What do you think would happen if the practice charged for medical
records, but Sara was unable to pay? Could she be denied her
information? Does a fee like this create barriers to care or other
inequities? You are required to submit original content.
