Table 2 Learning Goals—Describes how you will grow as a
professional.
• Public Health Domain: These are the 9
Undergraduate Public Health Domains designated by the Association
of Schools and Programs of Public Health (ASPPH). You can find the
list here:

You must address at least 3 different domains.
You can list a domain more than once (for 2+ goals).
• Learning Goal: Description of a public health
skill or field of knowledge that you would like to practice or
expand. You must select at least 5 learning goals for your
proposal.
• Learning Objective: Each objective must be
SMART: Specific, Measurable, Achievable, Relevant, Time-bound.
Objectives must address the related goal. You may have more than
one objective per goal.
• Expected Products: What evidence will you
have of your effort when the project is complete? What items can
you put in a portfolio to show your abilities to a potential
employer?
Table 3 Programmatic Goals—Describes how you
will serve the organization or community.
• Healthy People Goal: Go to this webpage:
https://www.healthypeople.gov/2020/topics-objectives. Select a
topic from the page. When you click the topic link, the new page
will tell you what the Healthy People programmatic goal is for that
topic. For example, for Diabetes, the programmatic goal is: “Reduce
the disease burden of diabetes mellitus (DM) and improve the
quality of life for all persons who have, or are at risk for, DM.”
You can copy and paste that goal into Table 3. You should have at
least 3 programmatic goals in your table.
• Programmatic Objective: Each objective must
be SMART: Specific, Measurable, Achievable, Relevant, Time-bound.
You must have at least 5 objectives. Each objective must address
the related goal. You may have more than one objective per
goal.
• Primary Tasks and Duties: What are the
specific actions you must complete to accomplish the objective?
• Expected Products: What evidence will you
have of your effort when the project is complete?
