CASE 14-12 Operative Report, Breast Biopsy with Needle Localization This patient presents for the removal of a breast lesion that was previously marked prior to surgery with a radiological marker. The CRNA independently performed the Monitored Anesthesia Care. She is an otherwise healthy woman. LOCATION: Outpatient, Hospital PATIENT: Marilyn Agnes SURGEON: Gary Sanchez, MD PREOPERATIVE DIAGNOSIS: Right breast microcalcification by mammogram POSTOPERATIVE DIAGNOSIS: Right breast microcalcification by mammogram PROCEDURE PERFORMED: Right breast biopsy with needle localization ANESTHESIA: 1% Xylocaine local; 16 cc (cubic centimeter) was used. The patient also received IV (intravenous) sedation. INDICATIONS FOR SURGERY: The patient is a 77-year-old white female who had undergone mammography. The patient was found to have microcalcifications in her right breast. The patient was taken to the operating room for biopsy. DESCRIPTION OF PROCEDURE. The patient had previously undergone needle localization on the right breast microcalcification. She was brought back to the operating room. The patient was then prepped and draped in the usual manner. Xylocaine was used as local anesthesia; a total of 16 cc was used. The patient also received IV sedation. An incision was made over the guidewire. The guidewire and surrounding breast tissue were incised and sent to radiology Radiology confirmed that the microcalcifications had been removed. Hemostasis was obtained using Bovie cautery. The operative area was thoroughly irrigated. The incision was then closed with figure-of- eight 2-0 chromic sutures for the deep and superficial layers. The skin was closed with 4-0 Vicryl subcuticular stitch, and Steri-Strips were applied. The patient tolerated the operation and returned to recovery in stable condition Pathology Report Later Indicated: Focal fibrosis with coarse microcalcifications CRNA CODE: QUALIFYING CIRCUMSTANCES CODE: (Answers to every other Case are located in Appendix E.
