An abandoned above ground industrial waste pit is located
adjacent to a creek that serves as the main source of potable water
for town A. The waste pit is supposedly 30 to 40 years old, and
holds approximately 10M gallons of unknown hazardous waste behind
earthen embankment. The mayor of the town has lately been very
concerned about the integrity of the embankment and contacted FDEP
for assistance. As a Six Sigma expert you have been hired by the
town to run an FMEA to understand the vulnerabilities, and
prioritize potential actions to prevent a disaster. Develop a
