Passage:
Nicole is an 18-year-old Caucasian female. She is 5’9” and
currently weighs 110 lbs. All her life she has dreamed of becoming
a model, and she is working hard to pursue that dream. She knows
that to become a top model, she must keep a very strict diet and
exercise every to keep from gaining weight. Nicole’s friends have
mentioned to her they are concerned about her eating patterns, but
Nicole just brushes them off – thinking they are probably just
jealous of the self-control she has over her eating habits.
Nicole eats about 500-700 kcals a day, focusing on fruit,
vegetables, and lean protein. However, her agent recently commented
that her thighs seem to have gotten larger, so Nicole has reduced
her dietary intake to just vegetables – the non-starchy kind- and
only drinks water with lemon to keep her metabolism going. She’s
determined to look perfect for her upcoming photo shoot. She has
also started fasting twice a month because she heard it was great
for weight loss. Her fasts last for 24 hours and she only drinks
detox tea and water with lemon during the fast. She loves the way
it makes her feel, so she is thinking of incorporating it into her
weekly routine. Nicole thinks fasting once a week will be a great
way to shed those last 10 pounds.
Nicole is very strict about her exercise regime. She hits the
gym seven days a week, and runs at least five miles on the
treadmill, sometimes more if she indulged in too many kcals the day
before. She will run an extra mile or two if she ate too much to be
sure she doesn’t gain weight.
Nicole hasn’t had a menstrual period in the last year, but she
is okay with this since bloating would ruin her photo shoot. She
always seems to be cold, so she brings her jacket everywhere she
goes. She also has noticed some weird, fine hair popping up all
over her body, and the hair on her scalp is becoming brittle,
breaking off, and falling out. The changes in her hair have been
alarming to Nicole because she wants healthy-looking hair for her
photo shoots. She decided to visit a Registered Dietitian
Nutritionist (RDN) to determine if there are any specific vitamins
or supplements, she should be taking for her hair to be healthy and
shiny again. While there, the RDN requested that blood work be
done. Her blood work indicated elevated BUN levels, elevated
hepatic enzymes, and decreased magnesium levels. Nicole’s blood
pressure was 80/50.
Questions:

experiencing that could be due to her eating disorder. Also,
ifferentiate which concerns are physical, laboratory, and
medical.
4. Explain Medical Complications (physical, laboratory, and
medical) that Nicole could develop in the future if she continues
her eating patterns.
