What are the ownership and/or affiliation of the facility?
(i.e., proprietary, government, religious)
What organization(s) accredits the facility? What is the
accreditation status?
Does this facility provide any services other than acute care
services? (i.e., home health, rehabilitation, psychiatric,
outpatient clinic, etc.) Please list.
What is the current occupancy rate?
What are the annual volumes at the facility? (i.e., # of
discharges, # of outpatient visits, # of clinic visits, # of ER
visits, etc.)
HIM DEPARTMENT
Coding Department
How many FTEs are devoted to the coding area? Are all of the
coders credentialed? (Please list credentials, i.e., 2 RHIAs; 2
RHITs; 1 CCS; 2 CCS-P) etc.
How many coders work on site? How many coders work remote? What
requirements are there to be able to work remotely?
Describe the functions of the coding area.
Who manages/supervises the coding area? Does he/she supervise
other areas? If so, please list.
If the facility does not employ coders, does the facility
contract for coding services?
Other Functions of the HIM Department
What other functions are performed in the HIM department? How
many FTE’s
are devoted to the other functions. List the functions and the
credentials of those
employees. ie. ROI, Physician Credentialing etc.
FILING NUMBERING SYSTEMS
What type of record filing system is used? Paper, Electronic,
Hybrid? Please describe.
What type of record numbering system is used for paper
records?
RECORD PROCESSING, CODING AND STATISTICS
Describe the flow of records from discharge to permanent file.
How are records
processed through chart assembly or verification to coding and
on to dropping the
chart to billing? Describe the process for paper charts vs.
electronic if both are used
What type of computer systems do the coders use? (i.e., encoder,
GROUPER, etc.)
What are the productivity and accuracy standards for coding?
What are the productivity standards for scanning paper charts to
EHR?
What are the steps in the ROI process? Start when the request
comes into the department and finish with providing the record to
the appropriate party.
Approximately, how many ROI requests are processed in one
month?
