2. JW has had her pain controlled in the inpatient unit with
hydromorphone 1mg SQ q3h and is not requiring any breakthrough
doses. She is getting ready to go home and she would like to take a
long acting oral medication She has hallucinations when she takes
morphine. What would you recommend to the physician and what would
you suggest for her breakthrough pain?
