PATIENT's CHART Information
======
Chief complaint: A 67 year old man
presents to the pulmonary clinic with history of COPD. Along with
the lung disease, he has developed pulmonary hypertension and has
been diagnosed with cor polmonale. 2+ pitting edema to the knees
and the lower extremities have been noted.
History:
Physical Exam: A thin elderly man who
appears older than his stated age
Vital Signs Pulse 110,
regular Respirations 28, labored BP
115/86 Temperature 37.5, oral
Skin: no lesions noted; prominent
wrinkling in the face and neck
Chest: increased AP
diameter; hyperresonant with poor diaphragmatic excursion (1
cm); decreased breath sounds throughout without wheezing or
crackles
Extremities: decreased pulses throughout;
no bruises; no cyanosis; edema present as noted
