Pressure injuries are a potential risk to client safety that
exist in the health care setting. Develop a nursing care plan to
prevent the likelihood of a client developing a pressure injury in
hospital.
Medical diagnosis / assessment findings: -
Patient is in hospital for extended period of time post-surgery.
Patient is ambulant with 4ww, has moderate dietary intake however
has type 2 diabetes. Patient wears glasses for long sighted vision
and has bilateral hearing aids. Patient has medical diagnoses of
OA, RA, Osteoporosis and HT
